making own tie rods RH + LH or RH only

   OK, I just gotta ask: Has anyone had a tie rod's jam nuts, (it has to be both of them for the rod end to un-tread from the tie rod), come loose letting the tie rod to drop off the rig? I have not heard of this happening. Has it become a problem lately?

I am just wondering if this is a solution looking for a problem, or is it a real life issue?
